Addresses the management of complex and critical needs of patients and their families across the continua of care. The focus will be on people with multiple chronic conditions experienced by various populations such as age groups, disease/health conditions, and settings. In addition, transitional care, critical care concepts, palliative, and end-of-life care will be explored.
